Bullet over bolt - FirearmsTrainingCentral - 03-19-2018

This is the first time this has ever happened to me. I went to charge the gun, slapped the mag in on an open bolt, and hit the bolt release and ended up with this

oops

What typically causes this? I really don't get malfunctions on this upper, other than the occasional failure to fully feed due to dirty bolt because I'm running suppressed. This is the first atypical malfunction I've ever had with this gun.

I have never seen that happen before.

Could be that when you "slapped the magazine in on an open bolt" the top round launched.

I think it's possible.  Maybe time to inspect that mag.
